Sign in sheet

This document is a standard sign in sheet for any meeting around CAM. This document can be blank, or it can be filled in prior to the meeting with attendees initialing their information on arrival

Agenda

This document is a template for a proposed agenda for the first facilitated meeting. Times and order of the agenda may vary depending on the total length of the community meeting and the goals of the community.

Evaluation

This document is a template of the evaluation process for the day, including a purpose statement for the meeting.

Write up of outcomes from the facilitated meeting

This document is a template of the ‘product’ that is generated for the community after the facilitated meeting. It compiles all of the discussion that was generated during the facilitated meeting and any notes that were taken. It serves as a place for further community activity.

Performance Rating Scale

This document is a sample of a rating scale that is useful for all stages of community development. A healthy coalition will have ratings closer and closer to 10 as it evolves. A new coalition would have many items with low ratings, and this can serve as a baseline. The goal is for a community to take this scale each year. It is distributed both electronically and at meetings. The key is to gather a measurable number of responses within a 30 day window.
