Please help evaluate the effectiveness of Child Health Notes for community users! The evaluation period goes through March 31, 2010. We will provide CHN coordinators with a summary of survey responses received specifically from readers in your county.

The most recent CHNs- Developmental Screening and Surveillance, includes a box inviting readers to respond to the CHN survey. Depending on how you distribute CHNs in your community you may wish to consider one or more of these steps to assure survey responses from your readers:

1. For electronic CHNs include language with a link to the brief online survey such as is already in the developmental screening CHN. Considering adding a personalized request from you or your organization encouraging readers to complete the survey.
2. For paper copies of CHNs, include a paper copy of the survey . You can make copies of the survey yourself to include or request them from Kate Orville. Self-addressed stamped envelopes are also available from Kate (the pre-printed envelopes have a bar code that will charge postage to the central medical home project only when and if they are mailed so feel free to request as many as you need).
